(1) SELF MASSAGE

The first thing we need to do is decrease tension in your injured piriformis muscle. The best way to do this is through some gentle self-massaging of the gluteus maximus, gluteus medius, IT band, and the piriformis muscle itself. When we decrease the tension in the gluteus maximus and medius, and the IT band, this in turn will decrease tension and stress on the piriformis muscle. These gentle self massage movements are the perfect way to start our program because they provide some immediate pain relief for you, and they build a solid foundation for doing the rest of the program.

(2) STATIC STRETCHING

With Piriformis Syndrome, the piriformis muscle tends to contract and shorten in order to protect the nearby hip joint, and protect the body from further damage. But this contraction and shortening puts additional stress on the hip joint and the piriformis muscle itself, and actually slows down recovery and prevention of further injury. So our goal with Static Stretching is to safely elongate the piriformis muscle so we can speed your recovery and prevent further injury. We’ll also statically stretch the hamstrings because they can shorten too, as a result of the piriformis muscle shortening.

(3) HIP FLEXORS

Most of the time with buttock pain, Piriformis Syndrome and Sciatica, you’ll find that the hip flexor muscles have also contracted and shortened as a result of the pain and injury. Just like we did with the piriformis muscle and the hamstrings, we want to safely and gently stretch and elongate the hip flexor muscle, so it can return to its proper length and full capability, which will decrease stress and tension on the piriformis muscle. When the piriformis muscle has less stress and tension, it can heal, return to its proper function, and stop agitating the sciatica nerve, which causes sciatica and buttock pain.

(4) DYNAMIC STRETCHING

When you have buttock pain, Piriformis Syndrome and Sciatica, most of the time the internal and external rotators of the hip will tighten up and that tightness inhibits recovery and prevention. Dynamic stretching is moving your muscles to form a stretch, but not holding it in the end position (like you do with static stretching). In this approach, we will dynamically stretch the internal and external rotators of your hip so we can loosen the hip and take stress and tension off of your piriformis muscle. In this approach, you’ll also get some dynamic pelvis stretching and low back stretching.

(5) GLUTEUS MAXIMUS

When you experience pain in your buttocks, hip or piriformis muscle, a lot of the time this will inhibit or deactivate your gluteus maximus muscles, which means they will stop doing their job properly - they won’t activate when they are needed. This puts a lot more pressure and stress on the piriformis muscles, which makes it more difficult to heal your Piriformis Syndrome and buttock pain, and can cause other problems in your body too. Here, our goal is to reactivate the gluteus maximus muscles and increase their strength and endurance. When this happens, you can heal your Piriformis Syndrome, buttock pain and Sciatica faster, and prevent further injury and pain in other areas of the body.

(6) CORE

Most people who have buttock pain, Piriformis Syndrome and Sciatica also have poor activation, endurance and strength in their core - their abdominal muscles. This puts more stress on the piriformis muscle, which makes it more difficult to heal. In this approach, we’ll work on activating, strengthening and building endurance in your core, which will help reduce and eliminate your buttock, low back and leg pain FASTER and prevent future problems

(7) HIP ROTATION STRENGTH

In addition to loosening the internal and external hip rotators (which we do in the Dynamic Stretching approach), we also need to strengthen them. When we increase their strength, they can stabilize the hip and support the piriformis muscle more, which will reduce your buttock pain, piriformis pain and sciatica pain.

(8) SITTING TO STANDING

With this approach, we want to incorporate some real-world movement - the movement we do when we go from sitting to standing. Various muscles that are affected when we have buttock pain, piriformis pain and sciatica pain - like the piriformis muscle, the gluteus maximus, and the gluteus medius, play a role in the sitting-to-standing movement, so the exercises we’ll do here will give us a chance to activate and strengthen those muscles within a real-world context, so we can reduce and eliminate your pain and get your body back to doing regular, basic movements with ease and comfort instead of pain and discomfort.

(9) LUNGE / GLUTEUS MEDIUS

The gluteus medius muscle is often overlooked, but plays an important role in overcoming hip and piriformis injuries and pain. These lunges and other exercises will help us activate, strengthen and lengthen the gluteus medius muscles, so we can heal your piriformis muscles and reduce and eliminate your buttock, back and leg pain.

(10) SINGLE-LEG SQUATTING

When you have buttock pain, Piriformis Syndrome and Sciatica, some of the muscles in your hip area may be shortened, contracted and impaired. If this is the case, your hip may not be properly stabilized and supported, and so every time you stand on one leg you’re putting lots of stress on your hips. And remember: when you walk, run and climb stairs, you spend a lot of time balancing on one leg, so you may be putting a lot of stress on your hips without even knowing it. We need to make sure other injuries don’t occur in your hip due to your buttock, piriformis, and sciatica pain, so in this approach we’ll work on the activation, endurance and strength of the muscles around the hip joint. One of the most effective and efficient ways to do this is single-leg squatting.

(11) SQUATTING

With these squatting exercises, our goal is to strengthen the muscles around the hips and knees, and activate, strengthen and build endurance in the gluteus maximus and gluteus medius muscles, so the piriformis muscle will be properly supported and have less stress and tension, which will help it heal and prevent future problems. In the later stages of the program, we’ll also work on explosive movements which will help with the final recovery of your piriformis muscle.
